
If you have insurance for medical eye care or vision care, it is YOUR responsibility to contact your insurance company to determine your benefits and referral requirements.
Contact your insurance company BEFORE YOUR APPOINTMENT to obtain the following information:
I have an appointment on _______ (date) with Dr. _______. Is this a participating physician?
Do I have coverage for _______ condition? If so, what amount?
Am I required to get a referral for this visit?
Have I met my deductible? What is the amount of co-pay?
Do I have vision care coverage? If so, am I eligible at this time? Is Dr. _______ a participating physician?
Ask the name of the person with whom you speak.
Insurance Claims
If we participate with your medical insurance carrier and your policy covers the service you receive, you will need to pay only your deductible or co-pay amounts at the time of service. We will bill the insurance carrier for the remainder.
If you have a special vision care rider or insurance plan and we participate with that plan, we will file claims for you. You must be seen for routine preventive care to use the benefit of your vision care coverage.
If we do not participate with your plan, we may be able to file claims for you or assist you in filing your claim.

Denied Charges
You will be billed for any charges denied by your insurance company and you
are responsible for payment.


Medicare Claims
Kentucky Eye Care participates with the Federal Medicare Program and will file the claims for covered services.
We participate with Anthem and Humana Medicare Replacement plans.
Medicare Coverage
Medicare covers your exams when you have an eye disease such are cataracts, glaucoma, or being followed for diabetes. Certain other medical conditions may also be covered.
Medicare does NOT cover routine annual eye exams or the $25.00 refraction fee. The refraction is the part of the exam that measures the correction you need to see well. This is done in medical and non-medical cases alike and is a diagnostic measure performed to determine the health of your eyes.
Medicare also does NOT cover certain other supplies and services such as contact lenses, eyeglasses (except once after cataract removal), surgical supplies or injections.
Kentucky Eye Care cannot act as an intermediary between you and your insurance company. Please contact them directly for explanation of any denied charges.
Payment Policy
If you have NOT met your Medicare deductible,
full payment is due at the time of service.
You are also responsible for the $25.00 non-covered refraction fee on all exams which include a refraction.
